The death of a construction worker who fell 45 feet while
performing repairs on a Philadelphia church roof in June has led to 10 safety
violation citations, including three for allegedly willful violations, against
James J. McCullagh Roofing Co. in Jenkintown. The U.S. Labor Department's
Occupational Safety and Health Administration cited the company with failure to
provide adequate fall protections and fined it $71,600, which it can appeal.
